A retrospective analysis of the results of complex treatment of patients mechanical icterus (MI) of neoplastic genesis was done. It was revealed, that the use of antegrade percutaneous drainage for the purpose of biliary decompression with the proper technical level and adequate preoperative preparation is accompanied by a relatively low percentage of complications and mortality. The clinical experience we have presented of using two-stage surgical treatment tactics for MI of neoplastic genesis allows us to recommend its widespread use in the practice of urgent surgical clinics

Successes of molecular biology adducted for new information about biological distinguishes of A hepatitis virus (HAV). Totally 6 genotypes of HAV had been founded, and each has several subtypes and area of geographical spreading. Purpose of this study of evaluation of HAV genotypes in Republic of Sakha (Yakutia). Materials and methods: Patients with HAV, who hospitalized in Yakutsk city clinical hospital and from Neryungri district was involved in this study. For diagnostics clinic-epidemiological, biochemical, serological and molecule-genetical (PCR) methods of HAV-RNA evaluation with recognition of genotype has been used. Results: Spreading of HAV subtypes in Yakutia significantly different from another regions of Russia with prevalence of IA subtype. Most spreaded subtype, according our study, is a III genotype

The Republic of Sakha (Yakutia) is considered one of the most unfavorable territories of the Russian Federation for the prevalence of hemocontact viral hepatitis B, C and D, and their adverse outcomes - cirrhosis and primary liver cancer [1,4,5]. According to official statistics, in 2017, only 328 cases of chronic hepatitis B (CHB), (34.2 per 100,000 cases) and 501 cases of chronic hepatitis C (CHC) were registered (52.1 per 100,000 cases). In the structure of chronic viral hepatitis, the proportion of chronic hepatitis B and chronic hepatitis C in 2017 was 39.5 % and 60.5 %, respectively. In different parts of the republic, the incidence rates of viral hepatitis are unevenly distributed, which is related to the conditions of everyday life of the population and the remoteness of residents of the regions from the large settlements of the republic, provided with all kinds of medical and social assistance, including modern laboratories, so necessary for hard-to-reach areas of the RS(Y). Of great interest is the intrafamilial spread of viral hepatitis. In families of patients with chronic hepatitis B, the prevalence of infection was higher (77.6 %) than in the acute viral hepatitis B (39.7 %). And the risk of infection in family hearth with active replication of the hepatitis B virus was 1.5 times higher than in hearth without replication of the virus at the source of the infection. The intrafamilial distribution of HCV-infection was less intense. Most often, markers of viral hepatitis C were detected among the spouses of infected (in 35.1 ± 7.9 % of cases). It is necessary to further modernize the system of providing the population with quality medical care, the implementation of national programs and directions for vaccination of the population against viral hepatitis B, early detection and timely antiviral treatment of patients with chronic viral hepatitis B, C and D for the prevention of cirrhosis and liver cancer, improving epidemiological surveillance for parenteral viral hepatitis in the region and dynamic monitoring of patients and their families

The article is devoted to the epidemiology and specific prevention of hepatitis A in the Republic of Sakha (Yakutia). It presents the data on the incidence of hepatitis A for the period 2000-2017. It is shown that vaccination against hepatitis A is the most effective strategy to control this infection in modern conditions. The aim of this work was to assess the epidemiological situation with hepatitis A in the Republic of Sakha (Yakutia). Materials and methods. The paper uses the materials of official statistics of the Territorial administration of Rospotrebnadzor of the Republic of Sakha (Yakutia) and the data of the Department for the treatment of patients with viral hepatitis SBU RS (I) “Yakutsk City Clinical Hospital”. Conclusions and results. As a result of selective immunization of risk groups over the past decades, the republic has managed to reduce the incidence of viral hepatitis A in general. However, in 2017, 34 cases of viral hepatitis A were registered; compared to the previous year, the incidence rate increased by 3.8 times. The incidence rate was 3.54 per 100 thousand population, which is 1.5 times lower than the average of Russia (5.52 per 100 thousand population). Also, routine vaccination against viral hepatitis A in children aged 20 months within the regional calendar of preventive vaccinations continues. With the increase in the number of vaccinated against hepatitis A, the incidence fell to occasional cases

In recent years, hepatocellular carcinoma becomes a serious problem in clinical oncology and hepatology due to the progressive increase in its frequency and the difficulties of early diagnosis. The diagnostics of the initial stage of liver lesions is difficult primarily due to the meager symptomatology and lack of informative general clinical methods of research. This article describes the clinical case of chronic viral hepatitis D, which led to the development of hepatocellular carcinoma in a youngpatient from the Republic of Sakha (Yakutia). The degree of severity of the course of hepatocellular carcinoma of the viral etiology depends on the activity of hepatic tissue damage by hepatitis viruses, the presence of active replication markers, and the signs of liver function decompensation due to the spread of the tumor process. This case dictates the need for mandatory examination of people with chronic viral hepatitis, as a risk group for the development of cirrhosis and liver cancer
